IMPORTANT Bug in the Waiting Room

Today we found a rather nasty bug in the code that controls the way the Waiting Room works. If you have completed the form in the WR but have never received a confirmation email or have not been added as a user please fill in the form again.

The boxes below show a range of grays from pure black to pure white. You should be able to see a clear difference between each shade of gray - if not please adjust your screens brightness and contrast.
